Primary Health Care for Spinal Cord Injury
The Ontario Neurotrauma Foundation (ONF) has funded a pilot project to identify how best to build capacity for the care of persons with Spinal Cord Injury in primary care. The main objective of this project is to assist persons with a SCI receive the right care, at the right time, in the right setting, through a collaboration between primary care providers and the Regional Spinal Cord Team at Hamilton Health Sciences (HHS).
This project helps to build capacity for care of persons with a SCI in primary care through support by a Nurse Clinician with extensive training and experience working with persons with SCI. The SCI Nurse Clinician:
