Patients Served
The clinic serves adult patients who are diagnosed with diabetes; specifically patients who require either specialist follow up with a physician specializing in diabetes or diabetes education.
Services Provided
Diabetes Care and Research provides consultation and education services to patients.
A translator can be provided if required.
The Diabetes Care and Research Team
The Diabetes Care and Research team is multidisciplinary consisting of Registered Nurses and Registered Dietitian Educators. As Hamilton Health Sciences is affiliated with McMaster University, learners may be present in the clinic and working with team members.
Your first visit to Diabetes Care and Research
At the McMaster site patients are seen by a Nurse Practitioner on the first visit followed by an endocrinologist at a later date. Follow-up appointments are booked with a nurse and dietitian. All patients will be triaged for timing and urgency of their first visit. Patients will be assigned to attend classes to meet their learning and diabetes management needs.
Patients should bring their medications, home glucose monitors and logbooks to each visit.
All patients must present their Health Card at each clinic appointment. Please ensure you health card is up to date with current address and has not expired. If you require assistance obtaining or renewing your Health Card Information please visit the Hamilton Ministry of Health Office.
