How do I make an FOI Request?
In many cases, it may not be necessary to make a formal request to get access to hospital records. Requests for information can be made by calling or visiting this web site for links to information publicly available.
When information is not available through this web site, a request can be made in writing or by completing the Access Request Form and sending it to the Freedom of Information Specialist at Hamilton Health Sciences. You will need to include an application fee of $5.00 - a cheque or money order payable to "Hamilton Health Sciences." Do not mail cash.
Under FIPPA, an access request must be made in writing, be accompanied by the $5.00 application fee, and provide sufficient detail to enable the hospital to identify the record(s) requested.
