Welcome to the Centre for Healthy Aging, located at St. Peter's Hospital in Hamilton, Ontario.
The Centre for Healthy Aging is a one-stop destination focusing on the health care needs of seniors who come to Hamilton Health Sciences for treatment of diseases and disabilities related to aging. The unique care needs of older persons often requires clinicians with specialized training in Geriatric Medicine and Geriatric Psychiatry, and the Centre for Healthy Aging is the “heart” for such care.
In the Centre for Healthy Aging, we are committed to delivering high quality care and an excellent experience to the patients and families we serve. As leaders in seniors’ care, our specialized teams of geriatricians and geriatric psychiatrists, interdisciplinary case managers, nurses, occupational therapists, chiropodists, intake and administrative professionals, and learners work together to ensure that we deliver evidence based, person-centred care throughout our outpatient clinics and outreach programs. We offer a shared care model with the primary physician and other service providers. The goal is to maintain this complex patient population in their home environment.
