Regional Rehabilitation Outpatient ServicesChedoke Hospital, Wilcox Building, First and Second Floors Phone: (905) 521-2100, extension 40801 or 40802 WHO WE ARE: Specialized Outpatient Rehabilitation Services (SORS) THE SORS TEAM: is made up of committed health care providers who will work closely with you to make sure your needs are met, and that you are able to safely rejoin the community as fully as possible. This Team may include: Physiotherapists, Physiotherapist Assistants, Occupational Therapist, Occupational Therapist Assistants, Speech Language Pathologist, Social Worker, Registered Dietician, and Recreational Therapist. Consultation with other healthcare providers may be arranged i.e. nursing. PATIENTS SERVED: SORS helps patients who have neurological conditions, such as stroke, acquired brain injuries, spinal cord injuries, and complex musculoskeletal conditions before and after a joint replacement, such as a hip or knee and seniors. SERVICES PROVIDED: We provide care to seniors 65+ year of age who have multiple physical, psychological, and social issues (e.g. dementia, safety issues, falls, and caregiver stress).
